With "Custom Ship Methods with Fixed Amounts" set, we've now been able to get the shipping methods we've specified listed. However, if the user changes their "ship to" country, when the form comes back it's lost all of the shipping methods! Any ideas how to correct this?

Also, the "checkout" button is then disabled, which is probably related to the first issue.

I tried it in my test portal and I have default country set to US and it worked but I think you are from the UK and set your home country to UK? Can you post a screen shot of how you configured your cart settings so we can try to replicate?
