Shipping Is Not Being Calculated

We are having trouble with a cart running 4.0. We are using custom shipping methods and free shipping for orders over $75.00. Everything works fine unless you go to checkout, with items totaling over $75.00, calculate shipping, remove items from the cart to drop under $75.00 total. Shipping then turns to TBD and lets you checkout. At the confirm page a shipping method is selected but shipping is labeled $0.00. Customer is very unhappy with multiple "Free Shipping" orders being processed. Need help on this one.
