If you click on the settings for the mini-cart module and expand Advanced Settings, you will see a check box for adding the module to all pages. If you already have the module in the header, then checking that box will add the mdule to the header on all your pages.

You can add the mini cart to any pane on any page in your portal. If you have a pane defined in your top header area then you can add it there. If there is no pane where you want to add it you would need to modify your skin to add a pane where you want the mini cart.
