Just to close this issue, we had determined that the issue was, we having quantity discounts configured from 1-5, 6-10, 11-15, etc. and you had a customer that always orders 48. Since she surpassed the quantity discounts, the cart charged her full price.

The solution would be to have quantity discounts that are as follows:

1-5
6-10
11+

So that if they order anywhere from 11 to 10000 of a product, the quantity discount is applied which can be done by entering 10000 or even 99999 to the Quantity Discount "To" field if you would like the discount to be applied to all purchases with a quantity of 11 or more.
