I'd like to offer free Domestic shipping (USA) only. Also foreign shipping seems to be under the real price by $2- to $3 each time.
Maybe an option for free shipping (zones?) and shipping and handling on certain zones? to make up for this issue?

There are a few different ways you could configure free shipping for domestic orders and calculate shipping for international orders.
1. In the cart admin settings check combine shipping and handling and configure the following:
a. Free shipping
b. Domestic handling charge = 0
c. International handling = any amount you want
2. In the cart admin settings configure custom ship methods by zip code and just define international zip code shipping costs.
3. We could customize one of the shipping setup screens and add the ability to configure ship code by country if you are interested in a quote.
