If a customer attempts to purchase, and the transaction is declined (e.g. expired card), then the customer tries again and IS successful on the second attempt, then a confirmation email is not sent.

Can you login to our test site http://www21.smith-consulting.com/ and try to replicate the issue. This will determine if the issue is with the latest version of the Cart or an issue with your specific dnn portal/cart install.
