Shipping by Quantity - at checkout Ship Method dropdown empty

I am using v3.88 and trying to setup Shipping by Quantity, but so far unsuccessfully. All efforts fail to populate the Shipping Method dropdown at the checkout page.

I have selected Shipping by Quantity as the Shipping Method in the cart setup. I have then setup a grid within Shipping in the Store Admin Menu.

Is there something else I should be doing?

In order to determine if the issue is with the latest cart v3.95 or an issue with cart v3.88 can you login to our test portal and configure the cart settings to match your cart settings and try to replicate the issue.

I found that I had made an error selecting "Custom Shipping by Product Amount" instead of "Shipping by Product Amount" in the Buy Now setup. This carelessness cost me 3 days.

On another matter, what version will have the minimum order capabilities?, and how do I get my hands on that version. I have just bought v3.88 3 weeks or so ago.
